Effective date: October 21, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our commitment to your privacy

We are required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach of your unsecured health information occurs.

Your rights

When it comes to your health information, you have certain rights. You may:

  • Get a copy of your health and billing records. Ask us, in writing, to see or get a paper or electronic copy. We will usually provide it within 30 days. We may charge a reasonable, cost-based fee.
  • Ask us to correct your records if you think they are incorrect or incomplete. We may say no, but we will tell you why in writing within 60 days.
  • Request confidential communications. You can ask us to contact you in a specific way (for example, a different phone or address). We will say yes to all reasonable requests.
  • Ask us to limit what we use or share. We are not required to agree, except that if you pay for a service or item out of pocket in full, you can ask us not to share that information with your health insurer for payment or operations, and we will say yes unless the law requires us to share it.
  • Get a list of those with whom we have shared your information for up to six years before the date you ask, other than for treatment, payment, operations and certain other disclosures. One list a year is free.
  • Get a copy of this notice at any time, on paper or electronically.
  • Choose someone to act for you. A person with your medical power of attorney or your legal guardian can exercise your rights.
  • File a complaint if you feel your rights have been violated (see "Questions and complaints" below). We will not retaliate against you for filing a complaint.

Your choices

For certain information, you can tell us your choices about what we share. You have the right and choice to tell us to share information with family, close friends or others involved in your care, or not to. If you are unable to tell us, for example if you are unconscious, we may share your information if we believe it is in your best interest.

We will never use or share your information for marketing or sell your information unless you give us written permission.

How we use and share your health information

  • Treatment. We use your information to provide your care and may share it with other professionals who are treating you, such as your physician or a referral for imaging.
  • Payment. We use and share your information to bill and get payment from health plans, auto insurance carriers, workers' compensation carriers or other payers.
  • Health care operations. We use your information to run our practice, improve your care and contact you when necessary.
  • Appointment reminders and follow-up. We may contact you by phone, text message or email about appointments, paperwork and your care, using the contact details you give us.
  • Business associates. Companies that help us (for example, our electronic health record, scheduling, texting and billing services) may receive your information. They must protect it under a written agreement.

We may also share your information when required or permitted by law, including: to help with public health and safety issues; for health research under strict conditions; to comply with the law, including with the Department of Health and Human Services; to respond to organ and tissue donation requests; to work with a medical examiner or funeral director; for workers' compensation claims, law enforcement purposes, health oversight agencies and special government functions; and to respond to lawsuits, legal actions, subpoenas and court orders.

Other uses

Other uses and disclosures not described in this notice will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.

Changes to this notice

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on request, in our office and on our website.
